Congratulations [member_first_name]! You’re one of the blessed and beloveds coming with Annie to Pura Vida Spa in February 2019 to relax, renew, have fun and do some sweet self-inquiry.
This form contains all the detail you need to complete your reservation. Annie will be sending emails as critical dates approach to ensure that you’re
participation is secured and that you are personally prepared for the retreat.

Costs & Payments
Your $400 deposit will hold your spot and go towards your final bill. The remainder of your bill is due: 12/1/2018.
Cancellations: If you need to cancel, you can do so up to 12/1/2017 and receive full refund minus a $75 admin fee. No refunds after that (if we can find someone to take your place, we will). Pure Vida will allow cancellations only if the San Jose airport is closed due to weather or other unforeseen event.
